摘要:[摘要] 目的:观察丁苯酞软胶囊治疗青中年缺血性卒中患者认知功能障碍的疗效,并初步探讨其作用机制。方法:80例青中年急性缺血性卒中合并认知功能障碍患者随机分为对照组和治疗组,每组各40例。对照组给予常规治疗,治疗组加服丁苯酞软胶囊,每次0.2 g,tid,20 d为1个疗程,间隔10 d,共3个疗程。治疗后1个月和3个月,比较2组患者简易精神状态评价(MMSE)量表评分、血清血管内皮生长因子(vascular endothelial growth factor,VEGF)及血管侧枝循环开放率。结果:治疗后1月和3月,2组患者MMSE评分明显提高,同组治疗前后比较差异有统计学意义(P<0.05),且治疗组MMSE评分增高程度明显高于同期对照组,治疗后1月组间比较差异无统计学意义(P>0.05),治疗后3月组间比较差异有统计学意义(P<0.05)。治疗后1月和3月,2组患者血清VEGF水平呈先升后降趋势,同组治疗前后比较差异有统计学意义(P<0.05),且治疗组上升幅度更明显,组间比较差异有统计学意义(P<0.05)。治疗后3月,对照组与治疗组侧枝循环开放率分别为57.5%和80%,组间比较差异有统计学意义(P<0.05)。结论:丁苯酞可有效改善青中年急性缺血性卒中患者的认知功能障碍,其作用机理可能与诱发内源性VEGF表达、促进新生血管形成有关。

Abstract:[Abstract] Objective: To study the effect of butylphthalide on cognitive impairment in young and middle-aged cerebral infarction, and to discuss its action mechanism. Methods: 80 cases of acute ischemic stroke in young and middle-aged patients with cognitive dysfunction were randomly divided into control group and treatment group, 40 cases in each group. The control group received routine treatment, and the drug group added butylphthalide soft capsules, with the frequency of 0.2 g per time, 3 times per day, 20 days for a course, interval of 10 days, and a total of 3 courses. After 1 month and 3 months of treatment, MMSE scores, serum VEGF and vascular collateral circulation rates were compared between the two groups. Results: After 1 month and 3 month of treatment, MMSE scores in the two groups were significantly increased, the differences were statistically significantly (P<0.05), the scores of the treatment group was significantly higher than that of control group, the differences of the two groups were no significant difference at 1 month (P>0.05), but were statistically significant difference (P<0.05) at 3 month. After treatment, the serum VEGF levels of 2 groups were increased firstly and then decreased, the differences of intro-group were statistically significant (P<0.05), and indexes in treatment group were significant increased than that in the control group, with significant difference between the two groups (P<0.05). After 3 months treat, the rates of collateral circulation in the control and treatment groups were 57.5% and 80%, respectively, and there were differences between the groups (P<0.05). Conclusion: Butyphthalide soft capsule could effectively improve cognitive impairment in young and middle-aged cerebral infarction, and its action mechanism may be related to the induction of endogenous VEGF expression and angiogenesis.
